Azevedo Family Psychology (AFP) is open and continues to provide most of our services through telehealth.
Dr. Azevedo currently offers only telehealth appointments and is accepting new clients.
To obtain services in person, you will be required to sign our INFORMED CONSENT FOR IN-PERSON SERVICES DURING COVID-19 PUBLIC HEALTH CRISIS FORM and agree to take certain precautions which will help keep everyone (you, your family, your AFP provider, and other patients) safer from exposure, sickness, and possible death. If you do not adhere to these safeguards, it may result in you starting/returning to telehealth appointments with your AFP provider.
AFP offices are taking the following office safety precautions during the Covid-19 pandemic to protect our patients and help slow the spread of the coronavirus: patients wear masks, AFP staff wear masks, AFP staff maintains safe distancing, physical contact is not permitted, and surface areas a disinfected 3 times daily.
